It's a hidden vehicle in ONS, you can unlock it somehow through the single player by playing as Mr. Crow on godlike I believe.
Or, in an ONS match offline you can summon it with: summon OnslaughtFull.ONSGenericSD

That's the only way I've ever gotten it. I always assumed it was just a hidden vehicle and the only way to get it is by summoning it, though what you said about Mr. Crow is probably right.

I played on at least one Server that had it enabled, but can't recall which one any more. Was funny as all get out to see those little toilet's haulin' keister across the map!

I remember reading in a few places about the Mr. Crow thing, because that is the only character where the name is said in the character selection screen. If my memory serves me right, doing single player with Mr. Crow unlocks a mutator. I'll have to do some Googling again to be sure.
Also if you're a server admin on a server with AdminPlus running, can summon it there too. I believe mappers can also stick it into custom maps.
Edit: After some quick Google searching, I guess I was wrong about it being unlockable. However there may be a mutator to stick it in, like the one for the ECE bonus vehicles.
