30 seconds of sitting around with no food and an empty beer bottle is enough to make me not tip the waiter/waitress. Sorry, but that's the way it goes. Keep my beer coming and you'll get a tip. If I gotta wait, too bad and I don't really care if I can drink beer faster than almost any human alive. This whole town knows that to begin with, so accomodate me already.

Tipping is a hidden tax, imposed by places that can't be bothered to pay their staff adequately. Seriously, when tips are a substantial portion of a persons wages, it's gotten out of hand. A sandwich shop can advertise a $5 sandwich, and then let you pay their staff the difference yourself from what it really should have cost.
From what I understand, Europe requires a living wage to be paid, of which tips are a *bonus* - which is, IMHO, a better system.
Regardless of the system, I will say, how much I tip is directly proportional to the quality of service. I've been known to cut that tip to 10% or less if the service stinks. I've also tipped at 20% or better when the service was excellent. I do not think that tips should be automatic - otherwise the establishment should just charge more, and pay their staff more. That said, I don't stiff on tips - unless the service was truly appallingly bad.
Call me a cheap bastard, but I still typically leave a 15% tip.

I usually just double the tax (comes out to 17-18%) and round up.

It isn't hidden. I also don't believe that it has anything to do with being "bothered" to adequately pay the staff. Tip-based jobs almost invariably mean that customers will receive better service than comparable non-tip paying positions.
What's the difference, really? Do you think you'd get the same quality service, no matter where you went, if restaurant owners were required to pay a minimum wage? Of course not--waiting tables simply isn't worth it for that little pay.
And I could be wrong, but I believe at least some other countries often charge a service fee, based upon the amount of the check. That wouldn't escape the tax-like quality of the tip, it would simply make it mandatory.

I tip all the services on wellfleation's list except the hotel cleaning service. I never have. I don't know of anyone that does.

The same, it's just never something I think about doing, since I'm hellbent on trying to get out the door before I'm charged for another night.
I do tip skycaps $10 every two bags, though. Maybe I'm just paranoid that if I don't tip enough that it won't get there?

The last time I flew I had a skycap put in way more effort that I would ever expect a skycap to in order to get us on an earlier flight because our scheduled flight was delayed a few hours and there were massive lines on the inside of the airport.

Oh, and FWIW, you never have to worry about tipping your cellular representative, but if you want to do something nice for us if we've done well, at least take the freakin' data package or other vertical service for a month and then drop it. Our mere existence depends more on attach rate of those services more than it does volume of sales, so if you've ever seen respectful, nice and knowledgeable associates shuffle out of your local store every 60 days, now you know why.
Obviously I'm rarely offered a tip because of the way the industry is, unless I'm doing some sort of unsanctioned repair work. Even then I'm sort of honor-bound to refuse it. One guy gave me and an employee Tennessee Titans tickets, though, which I just let the employee have so that she could take her husband.

The basic 15-20% tip is just part of the cost of going out to eat. It's ridiculous to consider it some sort of "extra" that the waiter has to work hard for.
It's nice to tip more on the occasions when the service is really good, or you're feeling extra generous, but that's just cheap to tip less just because the service was so-so.
I don't think I've ever had service that was so bad that I felt that it warranted no tip.

The entire concept behind making wait staff work for tips is all about ensuring they work their asses off for the customer.

Eh, not really. Taxes are a basic fact of life that you ought to get used to if you don't want to be bitter every time you look a paycheck.
