Need Help On a Double Accounts
Hi...
I don't know what my mom does to her computer, but this is the second time she has made two accounts (with the same name) on her iMac. So now she has two (same-named) "ForceQuit"s, "Restart"s, "ShutDown"s and "LogOut"s in her Apple Menu. When I click on an icon or radio button, I get a black lined box. I don't know how to get rid of one of these accounts and hopefully rid myself of the boxes. Oh! She also accidentally turned on speech. Can anyone help with that as well?

mmmmmm
The first thing I would do is get a new backup Firewire drive and use Carbon Copy Cloner to clone the drive.
Then we can go in and determine which of the accounts is the real account. The next step ? Well, we have safe choices because you are (or will be) backed up.

No need to fret: go to System Preferences > Universal Access > Seeing and turn off VoiceOver. You may also want to go to Keyboard & Mouse > Keyboard Shortcuts and uncheck some of the Universal Access shortcuts to prevent them accidentally being turned on in the future.

You mean I can't just get rid of one of them???!!! Well, I can't afford a new Firewire Drive and I'll bet she won't spring for one, so can I just back up important files to a CD and erase one of the accounts? Shouldn't each account still have the same settings? I'm mostly concerned with the internet settings...

It doesn't sound like there are two accounts, just that the Apple Menu is for whatever reason showing duplicate entries.
What accounts show up under the Accounts section of System Preferences?

That's why I came here. But before that, I did look Acccounts in SPs and could not find any additional accounts. Also, if I try to log out or shut down one of these "accounts" it does not respond. But what is and why would I have a dublicate entry??

I think it has something to Do with UNIX, as I have a screensaver (ShowOff) that shows doubles, sometimes triples, under running users.

Go to http://applejack.sf.net/ and follow the directions to install AppleJack on your computer. Once it's installed, reboot and press and hold Command (Apple) + S immediately upon hearing the startup chime.
Once the text on the screen stops scrolling (you should see a friendly prompt about AppleJack), type:
applejack AUTO reboot
This will initiate a deep clean of your system (leaving your files intact, but cleaning out all the excess rubbish that operating systems tend to accumulate) and reboot when it's done.
Give that a shot and see if it rectifies the problem.
Edit: it is important to capitalize the word AUTO. Else you will just get a standard clean.

Last time I was on this iMac the "double entries" were gone. Now I see, that something in the OS has been hosed. It may be Mia Culpa; I like to install new programs and "toys" all the time. I will try AppleJack and I'll do some other maintainance stuff. Last resort will be a clean install.
