Feedback PHP Script
A while ago I came to you guys asking how I could put a feedback form on my website to e-mail directly to my adress without opening up someone's mail client. I found the script but it obviously isn't working.
Here is the page:

As you can see, when trying to submit something you get a PHP error. Multiple, really. Can anyone figure out what's going on?

I didn't get a PHP error... it submitted a page to submit.php and then forwarded me back to feedback.html
You want to change the css on the input tags though, because right now it's a nice unreadable white on white.

Okay, so I got rid of the PHP errors, and added some new fields, but now, I just get a blank e-mail when someone submits feedback...

Can you post the source? Is mail() throwing an error? Are you setting the message correctly? Yadda yadda.
