I need to make a form that, when filled out, takes the text from the fields entered and displays it elsewhere in the document.

For example, if there are 3 fields for Street Name, City, and Zip Code, the text entered would be displayed later in the document as:

[Street Name]
[City], [Zip Code]

What program would work best for this?  Acrobat?  Word?  Spreadsheet?  Something else?
