The liver donor scene in The Meaning of Life always makes me grimace, that and the vomiting fat man.

As for horror movies, I'd have to go with the chainsaw up the crotch scene from the Texas Chainsaw Massacre remake, in the theatre everybody groaned, even the women.

When the Nazi Bastard is sloooooooowly pushing the blade into the GI who's name i don't know.

That makes me freakin' squirm, my body aches after watching that.

That's exactly the scene I was thinking of. That guy's name was Mellish (or something similar).

The withdrawal scenes in Trainspotting. Oh, boy.

Oh hell yeah.  Babies scare the crap out of me now.

Did anyone ever see the Dentist?  That movie was just plain horrible.  I never get squeemish, but when he starts drilling peoples teeth apart...
